CareOregon Advantage计划承保Medicare D部分下众多处方药品

欲了解您的处方药是否包含在计划中，请在我们的获批药品清单（也称为“处方集”）中进行搜索。

• Click on the formulary you want to access.
• 如果您正在访问PDF文件，点击Ctrl+F（电脑）或⌘-F（Mac），屏幕顶部将出现搜索栏。
• 在该搜索栏中输入药物名称并点击确定。
• If there are multiple similar results, you can use the Previous and Next buttons to look for the correct medication.
• 欲搜索更多内容，只需要在搜索栏中输入下一个药物名称并继续上述操作，直到您找到所有处方药。

Medicaid Formulary for CareOregon Advantage Dual Members

This list shows you what drugs are covered by Medicaid for members who have both Medicare and Medicaid through CareOregon, beyond the regular CareOregon Advantage formulary.
